- Humana announced it is partnering with Monogram Health to offer value-based kidney care in four states. Eligible Medicare Advantage (MA) HMO and PPO members in Tennessee, Alabama, Louisiana, and Mississippi can access Monogram’s specialized offerings, including in-home primary and specialty care, medication management, dialysis and transplant coordination, palliative care, and social services. As of June, more than 3.2 million of Humana’s MA members are cared for in 1,000 value-based arrangements across 46 states and Puerto Rico. (Press release here)
