Acclivus is a community health organization focused on the health and well-being of Chicago’s most vulnerable neighborhoods. One of Acclivus’ strategic activities is serving a network of Chicago-based hospitals treating patients injured by violence, performing a critical centralizing role in immediate violence intervention across the Chicago area.

Acclivus initially engaged with Third Horizon Strategies (THS) to explore opportunities to elevate their health care profile. As a community-based nonprofit entity, they were already engaged with some health care provider stakeholders, but the partnership with THS has allowed them to broaden their network of connections in Chicago and to develop a better knowledgebase of what’s possible in the broader health care ecosystem. 

You all have demonstrated that you are willing to come to the community, understand what the community’s experiencing, meet with us where it is best and most comfortable for us and understand our challenges that we have on the ground. And that’s sort of unique for a corporate boutique consulting firm to do.  That showed us the commitment you all have to community health, which we share.

The Health Care Council of Chicago (HC3), one of THS’ initiatives, has been able to activate unique partnerships with its members, including pro bono advisory services in finance and audits, contract development, and strategic communications. In addition to strategic introductions for external pro bono support, THS has provided 150+ hours in consultative and advisory support services.

As we’ve grown as an organization and as we continue to grow as an organization, we like utilizing and engaging with different corporate strategies that are sometimes more relevant and prevalent in the for-profit space and integrating those into a charitable cause, into the nonprofit space to maximize our impact and help us scale appropriately.  That’s what has been valuable for us, and I see that continuing as we grow and as you all grow.

Acclivus’ team recently embarked on a new project with THS’ newest division of data-support services to reimagine and redevelop their patient and community services database. Via the partnership, Acclivus hopes to manage and improve the quality outcomes and impact they have in the communities they serve.

Meghan is the Vice President of Community Impact at Third Horizon Strategies. Meghan is also the inaugural executive director of the Health Care Council of Chicago (HC3), an action-oriented collaborative that brings together leaders from across the health care system to address Chicago’s most important health-related issues. Meghan has over 10 years of experience in development, marketing, community engagement, corporate social responsibility, and philanthropy. Throughout her career she has worked in start-ups and nonprofits, developing programming and events, building sustainable relationships, and cultivating new and innovative ideas.
